Our Public Health Nurses collaborate with multiple divisions, community partners, and residents to reduce the risk for the spread of disease through case investigation, case management, TB case management, and various services offered.
- Communicable Disease Investigation
Public Health Nurses complete communicable disease investigations on reportable communicable disease for St. Joseph County residents in an effort to control and prevent further transmission.
Reporting communicable diseases to the Department of Health is mandated by Indiana Code 410 IAC.

Health care providers should print and complete the communicable disease report and fax the completed form to Nursing at (574) 245-6581.

- Lead Poisoning Prevention
- Rabies
Animal bites to a human victim are reportable according to Indiana Code 410 IAC. When reports of animal bites are received, the Nursing Department staff follow-up with bite victims to determine the possible recommendations for rabies prophylaxis and advise the victim of their possible need for medical care. A Public Health Nurse is available to offer guidance to the bite victim’s physician regarding rabies prophylaxis recommendations. Animal bite reports should be faxed to (574) 245-6581.
