Employee Benefits and Invest In Your Health
Employees on the State of Indiana health plan have access to several wellness programs, discounts, and rewards—many of which they might not know about. That’s why we want to make sure that as partner agencies, you are equipped with simple communications tools that you can use to show employees how they can get the most out of their plan. If you need additional promotional material, toolkits, or resources that are not provided here, please email spdcommuncations@spd.in.gov.
On this page, you will find promotional materials to help you drive engagement with the Wellness Rewards Program, Invest In Your Health branding and promotional language, as well as program descriptions for all programs available to employees.

Wellness Rewards Program
Program Overview: Employees and spouses eligible for the ActiveHealth program can each earn up to $500 ($1000 total) by each completing wellness activities. This year’s focus is on preventive care. For example, an employee can earn $200 in rewards for getting an annual physical and $50 for completing a dental exam + cleaning. Preventive activities like these are critical to understanding and maintaining health and offer more opportunities for early diagnosis of medical conditions.
To earn the reward, each activity must be visible in the Rewards Center on the ActiveHealth portal, www.myactivehealth.com/stateofindiana by December 31, 2024. Service or activity must be completed in 2024.

Optum Employee Assistance Program (EAP)
Program Overview: Optum’s team of experienced consultants is available by phone or online 24/7 to provide help with a range of life concerns and stressors, including: Relationship problems, workplace conflicts and changes, parenting and family issues, stress, anxiety, and depression, elder care support, legal and financial concerns. You have 8 free face-to-face EAP counseling visits with a licensed therapist available per issue, per year.

Government Center Clinic by Marathon Health
The Government Center Clinic is a specialized on-site clinic available to address your preventive, acute and chronic health care needs. It’s conveniently located in the Government Center south building and the visit fee for non-preventive visits is only $49. The visit fee will count toward your deductible and out of pocket maximum and health savings account dollars can be used to cover the cost of the visit.

Digital self-care resources and tools to help you learn to reduce stress, anxiety, depression, or substance abuse. MyStrength is a free tool provided by Anthem.

Anthem Diabetes Prevention Programs
Prediabetes often doesn’t cause symptoms, but it does increase the risk of developing type 2 diabetes, heart disease and stroke. Community Diabetes Prevention Programs
- Certified Diabetes Prevention Programs (DPP) are designed to help prevent or delay type 2 diabetes. These lifestyle change programs are offered face-to-face or virtually. Some Community DPPs are self-pay only and do not submit claims to insurance. If that applies to the Community DPP you’re participating in, you will need to upload a Community Based DPP Reward Form.
