Get rewarded for what matters! Employees and spouses eligible for the ActiveHealth program can each earn up to $500 in rewards. All you (and your covered spouse) have to do is complete wellness activities that make a real difference to your health.
Below are answers to common questions about ActiveHealth and the Wellness Rewards Program.
General
- Eligibility
Who is eligible to participate in the ActiveHealth program? The ActiveHealth program is open to State of Indiana employees, spouses, and adult dependents enrolled in a State Personnel Department sponsored medical plan. Employees and spouses who participate in the ActiveHealth program can earn wellness rewards.
Why do I have to be a member of the health plan to participate? The ActiveHealth program is a coordinated benefit within the health plan; therefore, it is only available to employees, spouses, and adult dependents who are enrolled in a health plan sponsored by the State Personnel Department.
Who can earn wellness rewards? Employees and spouses enrolled in a State Personnel Department sponsored medical plan can earn wellness rewards.
I am a new employee, can I participate? Yes, if you enroll in a health plan sponsored by the State Personnel Department. New employees are encouraged to begin participating in all wellness programs available to them. New employees typically have access to create an ActiveHealth account 1-3 weeks after health plan coverage begins. - Accessing the ActiveHealth portal
How do I create an ActiveHealth account? Go to www.myactivehealth.com/StateofIndiana and click on “Sign up for MyActiveHealth” at the bottom of the page. Enter your name as it appears on your insurance card, date of birth, and zip code.
If you get a message that says “Error. You are already registered with MyActiveHealth” you can use the Forgot Username or Password links to reset your information.
If you get a message that says “Error. We are unable to locate your information at this time.” either the information entered could not be matched, or ActiveHealth has not received your information yet. Verify that you have entered your information as it appears on your insurance card, and that it has been 1-3 weeks since your health plan coverage began. You can call the INSPD Benefits Hotline at 877-248-0007 to verify your health plan coverage start date.
Where do I go to login to the ActiveHealth Portal? Go to www.myactivehealth.com/StateofIndiana.
Is there an ActiveHealth app? Yes. You can get the app in the App Store or Google Play on your mobile device. If you have issues downloading the app, contact ActiveHealth Tech Support at (888) 588-8741, or email mobilesupport@aetna.com.
I have the ActiveHealth app, but it will not open on my phone. Make sure you’re using the latest version of the app. If you have an older version of the app, please download the updated version. What rewards are available through the ActiveHealth program? Employees and spouses eligible for the ActiveHealth program can each choose from more than 29 activities to earn up to $500 in gift card rewards by completing preventive care screenings and wellness activities.
How can I see if I have earned a reward? The Rewards Center in your ActiveHealth portal shows all rewards you have earned. Log in to the ActiveHealth portal: www.myactivehealth.com/StateofIndiana.
Do activities completed in 2024 count toward 2025 rewards? Only activities completed in 2025 are eligible for rewards.
Can I complete activities on state time? While you cannot complete activities on state time, you are encouraged to work with your manager to use the WHOLE policy in order to flex time to complete rewardable wellness activities. - Gift Cards
Who can earn gift card rewards? Employees and spouses who are eligible for the ActiveHealth program. To be eligible for ActiveHealth, you must be enrolled in a State Personnel Department sponsored medical plan.
What activities earn gift cards? In 2025, you can choose from more than 29 wellness activities to earn gift card rewards. This helpful chart summarizes the rewardable activities. Visit your ActiveHealth Rewards center to see full details on each activity by logging in at www.myactivehealth.com/StateofIndiana.
Is there a deadline to earn and redeem gift cards? Yes. All gift cards must be earned and redeemed by December 31, 2025. Keep in mind many activities have processing time that impacts how quickly the reward is shown as earned in the ActiveHealth portal. Please see the details included for each reward in the ActiveHealth Rewards Center for estimated processing times specific to each reward. Rewards must show earned in the portal by December 31, 2025 to be redeemable.
I completed an activity, but the reward does not show earned yet. What is going on? You may be checking too soon. Review the reward details in the ActiveHealth Rewards Center for estimated processing times specific to each reward. Processing time allows for review and verification to ensure reward requirements are completed. If it is outside the listed processing time, contact ActiveHealth customer service at 855-202-4219.
Can my dependents earn gift cards? No. While covered adult dependents can participate in the ActiveHealth program, they are unable to earn gift cards.
Will the gift cards be taxed? Yes, gift cards are taxable income based on IRS regulations. The value of the gift card will be included in your gross wages with a description of “Wellness Reward” on a paycheck following the redemption of the gift card.
What is the difference between earning and redeeming a gift card? An earned gift card means that you have completed an eligible activity and you are eligible to receive a gift card. A redeemed gift card means you have logged in to the ActiveHealth portal and used Rewards Center to select a retailer to receive your gift card.
How do I redeem my gift cards? Gift cards can be redeemed in the ActiveHealth portal in the Rewards Center. Click on “Redeem My Rewards” located under the progress tracker. Select your preferred retailer and the amount you’d like to redeem. The confirmation email will come from stateofindianareward@blackhawkrewards.com. The gift card code email will come from yourdigitalreward@blackhawkrewards.com.
Do I have to earn a minimum amount before I can redeem a gift card? Yes, you need to earn at least $50 to redeem your first gift card. Gift cards are only available in $50 increments up to the $500 maximum reward.
Is there a limit to the number of gift cards I can redeem? Yes, a limit of 10 gift cards can be redeemed per person per program year.
Who do I contact if I lost or did not receive my gift card? Call ActiveHealth’s technical assistance center (TAC) gift card support line at 800-491-2435 for lost or missing gift cards.
Preventive Care Activities
- Annual Physical
What is the wellness reward for completing an annual physical? Employees and spouses eligible for the ActiveHealth program can earn $200 for completing an annual physical with lab work and submitting the Annual Physical Results Form to ActiveHealth.
How long does it take for the activity to show as completed in the ActiveHealth Rewards Center? This activity will show as completed within four weeks of your Annual Physical Results Form being submitted to ActiveHealth and accepted for processing. To be accepted for processing, forms must be legible and fully completed including all required values and required signatures.
What if my health care provider doesn’t recommend lab tests for some of the values on the Annual Physical Results Form? If your provider determines some lab test values are not necessary for you, the provider can note that on the Annual Physical Results Form and the form will be accepted if noted properly Height, Weight, and Blood Pressure are always required to receive the reward.
What is an annual physical? It is an annual visit with your health care provider to assess your current health, identify any necessary preventive care, and review health changes over time.
What should I expect at an annual physical? At your annual physical, your provider will review your family and medical history, check your vital signs, order lab testing, and discuss lifestyle habits. Your provider will answer your health-related questions and offer guidance to improve your overall health and wellbeing.
How do I prepare for an annual physical? Create a list of all medications you currently take, including over-the-counter drugs and supplements. Include a list of any symptoms or pain you are experiencing and any questions you would like answered.
How do I find an in-network provider for an annual physical? The Anthem Health Guide team can help you find an in-network provider by calling 877-814-9709. The Anthem app, called Sydney, also has a provider search tool and an Anthem Health Guide chat option.
Why is an annual physical important? Your annual physical allows you to spend quality time with a provider to ask health-related questions, catch minor issues before they become problematic (e.g., cancers, heart disease, infections), and offers guidance to improve your overall health and well-being.
What is the deadline for earning the annual physical reward? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2025 to earn the reward. All gift cards must be redeemed by Dec. 31, 2025. Keep in mind processing time is up to four weeks from the date the form is accepted to when the activity is shown as completed in the ActiveHealth portal. Submit your form by November 30 to ensure it will be processed before the deadline.
Can my employer see this information? No. The annual physical is private and confidential. Your privacy is important to the State Personnel Department and to ActiveHealth.
How much does the annual physical cost? Preventive care is not subject to deductible or co-insurance. An annual physical is considered preventive care so there is no charge to you. Keep in mind providers are required to bill for all services provided during an appointment. If non-preventive care is provided at the same time as the annual physical, there could be out of pocket costs.
How do I get the reward for completing an annual physical? When you call to schedule your appointment, make sure to mention the lab values required on the Annual Physical Results Form. Print the Annual Physical Results Form and take it with you to your appointment. You fill out Section 1. Make sure you enter your name as it appears on your insurance card. Your provider fills out and signs Section 2.
Submit the completed form to ActiveHealth by fax or secure upload in the ActiveHealth portal. You will receive a confirmation when the form has been accepted. The activity will show as completed in the ActiveHealth Rewards Center once your form is processed. Form processing takes up to four weeks from the date your form is accepted. If you fax the form, keep your fax confirmation until the activity is shown as completed in the ActiveHealth portal.
Where is the Annual Physical Results Form for the annual physical? The Annual Physical Results Form is linked in the activity information in the ActiveHealth Rewards Center or can be downloaded here. - Cancer Screenings
What is the wellness reward for completing a cancer screening? Employees and spouses eligible for the ActiveHealth program can earn $50 for each cancer screening completed in 2025. Rewardable cancer screenings are breast cancer, cervical cancer, colon cancer, prostate cancer, and lung cancer.
How long does it take for the Cancer Screening activity to show as completed in the ActiveHealth Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem. The activity will show as completed within two to three weeks of your claim being processed by Anthem.
What cancer screenings are rewardable? Breast cancer, cervical cancer, colon cancer, prostate cancer, and lung cancer screenings are rewardable activities in 2025.
Who should get a cancer screening? Talk to your primary care provider to see which screenings are right for you. You can also visit health.gov/myhealthfinder to learn what screenings may be right for you.
Why are cancer screenings important? Early detection of cancer increases treatment options and odds of remission.
How do I find an in-network provider for a cancer screening? The Anthem Health Guide team can help you find an in-network provider by calling (877) 814-9709. The Anthem app, called Sydney, also has a provider search tool and an Anthem Health Guide chat option. You can also visit www.anthem.com/find-care to search for providers in your area.
What is the deadline for earning the cancer screening rewards? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2025 to earn the reward. Only screenings completed in 2025 are eligible for rewards. All gift cards must be redeemed by December 31, 2025. Keep in mind the cancer screening rewards are considered earned when the activity is shown as completed in the ActiveHealth portal.

We understand that not all screenings are right for everyone, and not all are recommended to be done annually. This is why there are lots of activity options to earn the $500 in gift card rewards.
How do I report that I’ve received a cancer screening? You do not need to report anything. The activity will show as completed within two to three weeks of your claim being processed by Anthem.
If you did not use your State Personnel Department sponsored health plan when receiving your screening, you will need to follow the Missing Preventive Services Form instructions. You can find the instructions and a link to the online form in the ActiveHealth Rewards Center in the activity tile for the screening that is missing. This form must be submitted by November 30 to allow for processing time.
Why isn’t my reward for my cancer screening showing in the ActiveHealth portal? You may be checking too soon. Cancer screening rewards are automatically completed within two to three weeks of your claim being processed by Anthem. Once you can see your claim has been processed at Anthem.com, expect to see your reward in ActiveHealth within two weeks.
Although uncommon, sometimes providers do not submit claims right away, or there is a problem with the way a claim was filed. If you’ve waited more than four weeks from the date of your cancer screening and Anthem still hasn’t processed the claim, you can use the Missing Preventive Care Service process to submit documentation. You can find the instructions and a link to the online form in the ActiveHealth Rewards Center in the activity tile for the screening that is missing. Documentation will not be processed unless it has been four weeks since the date of your service. This form must be submitted by November 30 to allow for processing time.
If you did not use your State Personnel Department sponsored insurance when receiving the service, you can use the same Missing Preventive Care Service process.

What is the wellness reward for completing a dental cleaning? Employees and spouses eligible for the ActiveHealth program earn $50 for completing a dental cleaning in 2025.
How long does it take for the Dental Cleaning activity to show as completed in the ActiveHealth Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem. The activity will show as earned within two to three weeks of your claim being processed by Anthem.
What is a dental cleaning? A dental cleaning includes an exam by a dentist and scaling and polishing your teeth to remove any plaque and tartar build up. It is an important part of maintaining oral health. Dental cleanings should be completed every six months.Who should get a dental cleaning? Why is a dental cleaning important? Getting regular dental cleanings can help find early warning signs of health problems and prevent dental problems.

What is an eye exam? An eye exam involves a series of tests to evaluate your vision and check for eye diseases. It is recommended that all adults complete an eye exam each year.
Who should get an eye exam? It is recommended that all adults complete an eye exam annually.
Why is an eye exam important? An eye exam can help detect eye problems at their earliest stage when they are most treatable.
How do I find an in-network vision provider? The Anthem Health Guide team can help you find an in-network provider by calling 877- 814-9709. What is the deadline for earning the eye exam reward? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2025 to earn the reward. What is the wellness reward for receiving a vaccine? Employees and spouses eligible for the ActiveHealth program can earn $25 for each rewardable vaccine received in 2025, up to $100.
How long does it take for the Vaccines activity to show as completed in the ActiveHealth Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem or CVS/Caremark. The activity will show as earned within two to three weeks of your claim being processed by Anthem or CVS/Caremark.
What vaccines are rewardable? COVID-19, flu, hepatitis A and B, HPV, measles, mumps and rubella (MMR), meningitis, pneumonia, chickenpox, shingles, diphtheria, tetanus, pertussis (whooping cough), and respiratory syncytial virus (RSV) are all rewardable vaccines in 2025.
Who should get a vaccine? Check with your provider to see which vaccines are right for you. You can review vaccine guidelines published by the Centers for Disease Control and Prevention (CDC) at www.cdc.gov/vaccines/imz-schedules/index.html or visit https://health.gov/myhealthfinder to see all preventive care recommended for you.
Why are vaccines important? Receiving vaccines is the best way to prevent illness.
Where do I go to get a vaccine? You can get a vaccine at a pharmacy, your provider’s office, or your county health department.
What is the deadline for earning the vaccine rewards? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2025 to earn the reward. Only vaccines received in 2025 are eligible for rewards. All gift cards must be redeemed by December 31, 2025. You will take a Prediabetes Risk Survey prior to starting the program to ensure it is a good fit for you.

The Health Assessment provides an easy-to-understand report so you can take action. What is the deadline for earning the health assessment reward? The activity must show as completed in the ActiveHealth Rewards Center by Dec. 31, 2025 to earn the reward. All gift cards must be redeemed by December 31, 2025.
Can my employer see this information? No. The health assessment is private and confidential. Your privacy is important to the State Personnel Department and to ActiveHealth.
Do I need lab results to complete the health assessment? No. You only need to know your height and weight. What type of credentials and certifications do the health coaches have? ActiveHealth’s team of health coaches is made up of qualified professionals like Certified Diabetes Educators, Registered Nurses, Registered Dieticians, Nutritionists, Certified Health Educators, and Exercise Physiologists. The team also includes a Diabetes Registered Nurse specializing in helping individuals understand recommended diabetes care and medications.
How do I schedule a Health Coaching appointment?- Health coaches are available Monday through Friday from 9 a.m. to 9 p.m. ET and Saturday from 9 a.m. to 2 p.m. ET, by appointment. To schedule an appointment, call (855) 202-4219.
- Check with your human resources department to see if onsite health coaching is available at your work location. These appointments are scheduled through work site sign-ups.

Select the topic that is right for you and click “Let’s Go” to register. You will receive a link in your email that you can use to join the sessions, or you can join from the ActiveHealth portal.
How does group coaching work? Join the group coaching session by clicking the link in the registration email you received, or through the coaching tab in the ActiveHealth portal. As the coach presents the content, there are opportunities for the group coaching participants to provide feedback to the group and ask questions through a private chat that only the coach can see. The coach then shares the feedback and answers the questions that are received with the group. This protects your privacy and allows the group to learn from each other.

A new heart earning week starts each Sunday.
How do I earn hearts through health goals? Go to Coaching and select Digital Coaching to start earning health goal hearts. You will be asked a series of questions to help set a health goal. You are then given quick daily activities to complete to help you meet that goal. Completing the daily activities earns you hearts up to a daily max of 120 hearts.
How do I earn hearts through health education modules? Go to Coaching and select Your Health Education to start earning health education module hearts. Browse the categories and click on a topic that interests you. Complete all the activities in that topic to earn 100 hearts up to a weekly max of 800 hearts.
How do I know what level I am on and how many hearts I have earned? The ActiveHealth homepage displays your level and total hearts earned in the top right portion of the webpage. You may need to refresh the page if you’ve just earned more hearts. - Physical Activity Tracking
What is the wellness reward for completing Physical Activity Tracking? Employees and spouses eligible for the ActiveHealth program can earn $15 for every 30 days at the goal activity level tracked by a device synced to the ActiveHealth portal in 2025, up to max of $180. The goal activity level is either 10,000 steps or 30 active minutes. Any day that meets one of these criteria counts towards the reward.
How long does it take for the Physical Activity Tracking activity to show as completed in the ActiveHealth Rewards Center? This activity will show as completed as soon as the tracker data showing each 30th day is received by ActiveHealth. You can verify what tracker data has been received by ActiveHealth on the ActiveHealth portal’s homepage.
How do I start tracking physical activity? Log in to the ActiveHealth portal and connect your device in the Tracker section on the homepage. Only data from a device connected to your ActiveHealth portal will count towards the completion of this activity. It is important to connect your device right away because only data tracked starting with your device connection date will sync to the ActiveHealth portal.
Do I have to use a fitness tracker for steps and/or active minutes to count towards the rewards? Yes. You must connect a fitness device to the ActiveHealth platform to earn the reward for physical activity tracking.
Will I be provided with a fitness tracker? The State of Indiana will not be providing devices; however, there are free apps, like RunKeeper, that will sync with ActiveHealth.
When I connect my fitness tracker, will all of my steps for the year sync? No. Only data from the day you connect your device and going forward will sync to your account.
