Request Records
Access to these records are strictly regulated and limited to authorized individuals and entities, in accordance with legal guidelines. Ensure that all information on the form is accurate and legible to avoid delays. Incomplete forms or request without the Authorization of Release Form will not be processed.
Authorization of Release Form
The representative of the incarcerated individual must fill out the State Form 46729 Authorization of Release form. This form must be fully completed and signed to authorized the release of records.
Submitting Offender Records Request
Once the Authorization of Release form has been completed, request may be submitted by either mail or email. Please be sure to include both the Request and Authorization of Release form in your message.
